GETTING READY FOR YOUR VISIT:

What to Bring With You

We ask that you bring your insurance card, photo ID and valid Visa or Master Card credit card with you to every visit. If you have previous medical records, please bring them as well or have them faxed or mailed to our office.

Your Registration Process

Please plan to arrive to your appointment 15 minutes early to complete the registration process. We ask that you complete our Online Health History Questionnaire and review our forms and policies. You will be asked to provide your signature of acknowledgement and acceptance of all these at your visit. You will also be asked to sign our HIPAA Acknowledgement, as required by law, so that you are aware of our privacy policies.

Your Payment

Please be prepared to pay your deductibles, co-pays, co-insurance or any other amounts due at the time of your visit. We accept MasterCard, Visa credit card and cash payments only. Please contact our billing office should you have any financial questions or concerns at 703-404-4401.

COSMETIC SERVICES:

In addition to all gynecological services, Dr. DeCosimo specializes in LASER spider vein treatments, LASER hair removal, LASER acne treatment, LASER skin rejuvenation procedures, Botox® / Dysport® and dermal fillers (Juvaderm®). Microdermabrasion and chemical peel treatments are also offered in our office.

You can expect to receive Aesthetic and Cosmetic services in a professional, discreet manner, delivered personally by Dr. DeCosimo. She is an active member of the American Association of Cosmetic Surgeons and she has been providing treatments for almost a decade, to women seeking to enhance their appearance through minimally invasive procedures. Surgical Services:

Dr. DeCosimo performs her surgical procedures at INOVA Fair Oaks Hospital or at the Northern Virginia Surgical Center– one of the newest state of the art outpatient surgical centers in our area. She also performs many minor surgical procedures at our office.

Minimally Invasive Surgery

Minimally invasive surgery (sometimes called laparoscopic surgery) is done through small incisions. Using specialized techniques, miniature cameras with microscopes, tiny fiber-optic flashlights and high definition monitors. These surgeries can be performed through incisions that require only a stitch or two to close. For patients, minimally invasive surgery means less trauma to the body, less blood loss, smaller surgical scars and less need for pain medication. Patients leave the hospital sooner after minimally invasive surgery and return to normal activities sooner than with conventional open surgery, eliminating large incisions and long surgical procedures Minimally invasive surgery is performed both on an inpatient (stay in the hospital) and outpatient (go home the same day) basis. surgery. By keeping the incisions the smallest possible, most patients can leave the hospital the same day, are back to work within 5 to 7 days, have minimal pain, and are active throughout their recovery period.

During Dr. DeCosimo's OB/GYN residency training at The Washington Hospital Center, in Washington, D.C., she was the recipient of the Outstanding Laparoendoscopic Resident Award in 1996 and has extensive training and experience in many conditions affecting women's health using advanced laparoscopic and minimally invasive surgical techniques for treatment of benign gynecologic conditions such as uterine fibroids, endometrial polyps, ovarian cysts, heavy periods, endometriosis, urinary incontinence, permanent sterilization.

Using the latest and highest level of laparoscopic technology with excellent gynecological surgical techniques, Dr. DeCosimo is confident that this is the logical, safest and most cost effective approach to most gynecological surgeries. With over 15 years of experience, she is confidently able to offer these surgical options to her patients.

BUSINESS OFFICE:

Our billing and accounting is handled by Inficare Medical Billing Services. The business office is available to patients Monday - Friday during office hours. We gladly accept cash, or VISA/MasterCard. If there is a problem with your billing, we appreciate you bringing it to our attention as promptly as possible. All billing and insurance inquiries should be directed to Inficare Services by calling their office at 703-404-4401.

Fee Information:

Our fees are within the customary range for this area and reflect the level of care you will receive and the high level of training by the physician. We have standardized charges for various procedures. These fees are listed in the office and are available to you at all times. If you have any questions about fees, please feel free to discuss them with our office.

Office visit copays or fees for which the patient is responsible must be paid at the time of the service. Patients will be charged their copay for any service performed in our office, whether performed by the physician or not, including laboratory procedures at the time of service.

Payment is expected at the time of your office visit, as mounting administrative and billing costs contribute to increased fees. Your credit card on file will be used to charge any amounts deemed due by you by your insurance company.

Insurance

Your insurance coverage benefits is a contract between you and your insurance company. You are responsible for your bill regardless of what your insurance pays. You may request a receipt which should be used for insurance purposes. Our office staff will file insurance claims for gynecological and surgical care. You are required to present our front desk staff with your insurance card at time of visit. If you do not have your insurance card with you, you will be asked to reschedule your appointment.

Please be sure to fill in your insurance information correctly, including group and identification numbers. We will help you in whatever way we can, but you can help us save time and serve you better if you make sure that your particular insurance policies cover the services for which you want to file claims. We will assist you making sure that your insurance pays us timely and appropriately for services we have provided to you. However, please be aware that ultimately, you are financially responsible for payment of bills incurred.

We also accept self pay and out of network patients and would be glad to assist you in arranging for payment of services.
